UnitedHealthcare 2027 Medicare Plans Shift Toward HMOs and SNPs
What UnitedHealthcare announced
UnitedHealthcare introduced its 2027 Medicare Advantage lineup on Oct. 1, before the Oct. 15 start of open enrollment. The company built the announcement around affordability, simpler plans and better-connected care, and it described itself as the most chosen Medicare brand for the 18th year in a row22. All members will keep $0 primary care visits, $0 preventive services and $0 Tier 1 prescription copays24. The insurer says more than 94% of Medicare-eligible people nationwide will be able to buy one of its Medicare plans24. It also says about 90% of people in its service areas will have a $0-premium option28.
The more important part of the release is where the company is steering members. UnitedHealthcare says it will continue to prioritize HMOs, Chronic Special Needs Plans and Dual Special Needs Plans in 202724. It says about 95% of Medicare-eligible people in its footprint will have an HMO option, most of them at a $0 premium. It also says HMO members saved more than $175 a year in out-of-pocket costs compared with members of local PPOs24. Its D-SNPs, which coordinate Medicare and Medicaid benefits, will reach about 74% of people eligible for both programs24.
The company also changed its cards. Members will get a separate health plan ID card, and the UCard will be used for benefit credits and rewards. The UCard now runs on Mastercard, and qualifying D-SNP members can use it to pay utility bills in real time24. Benefits can be spent at more than 70,000 retail locations, including Walmart, Dollar General and Target24. On drugs, the company points to a network of more than 60,000 pharmacies, 100-day supplies for many common medications, and $0 Tier 1 copays at preferred pharmacies for its stand-alone Part D plans24.
The fine print: a narrower plan with higher cost-sharing
The press release does not cover what the company dropped or cut back. UnitedHealthcare president Bobby Hunter said the company concentrated on the core benefits people use most35. He also said it will leave some areas where it sells a larger share of PPO plans36. About 390,000 members are affected by plan discontinuations, and UnitedHealthcare is leaving Wyoming entirely36. The share of its members who can choose both an HMO and a PPO falls to 66% from 70%36. Hunter blamed funding pressures, higher medical and drug costs, and increased utilization37.
The size of the pullback depends on what is being counted:
- Company count: Forbes, using the company's figures, reported that UnitedHealthcare's individual Medicare Advantage footprint grows by four counties, to 2,79135.
- Becker's: Its count, which leaves out special needs plans, shows UnitedHealthcare dropping from 2,597 counties to 2,457, a 5.4% decline34.
- KFF: Its analysis of plans that include drug coverage found UnitedHealth leaving 228 counties and entering nine33. It will offer those plans in about 70% of U.S. counties, down from nearly 80%37.
These numbers do not really contradict each other. They show that the company is cutting general-enrollment and PPO coverage while keeping special needs plans in many of the same counties. One analysis found that UnitedHealthcare is pulling standard plans from 140 counties and leaving most of them with only its special needs plans36.
Costs are also moving against members. A broker's analysis of published plan data found UnitedHealthcare premiums up about 9%, out-of-pocket maximums up about 11% and Part D deductibles up about 33%. It called that the steepest combination among the large national carriers25. These are third-party estimates, not company figures, but they cut against the affordability message. The same analysis found that UnitedHealthcare's share of members in plans rated four stars or higher slipped from 78% in 2026 to roughly two-thirds31.
The company's line that more than 95% of non-SNP members never reach their out-of-pocket limit24 is accurate. It also means that higher limits mostly affect the sickest members, who are the people least able to absorb them.
The wider Medicare Advantage market
UnitedHealthcare's moves fit the rest of the market. CMS projects the average Medicare Advantage premium will fall 16.5%, from $14.37 to $12.00 a month14. The agency also expects dental, vision and hearing benefits to stay stable overall14. In practice, that average saves about $2.37 a month17, and roughly two in ten enrollees cannot keep their current plan at the same or a lower premium19. The median in-network out-of-pocket limit in CMS's landscape file rose from $6,500 to $7,10012. An Oct. 1 Leerink Partners analysis found that many large carriers cut dental benefits and raised cost-sharing37, which is hard to square with CMS's description of stable benefits.
The pattern is that insurers kept advertised premiums low and raised costs elsewhere. One comparison guide said carriers protected the headline price by raising the out-of-pocket ceiling13. CMS raised Medicare Advantage payments by 2.48% for 202719. That is a modest increase for plans facing the cost trends described below.
County exits are hitting rural areas hardest. KFF counts 193 counties with no general-enrollment Medicare Advantage drug plan in 2027, up from 123. About 638,000 beneficiaries live in those counties33. MoneyGeek found that about one in five rural general-enrollment members was in a plan with no listed successor, compared with one in eight in metro areas32. Molina is leaving individual Medicare Advantage entirely34, and at least 11 carriers are exiting the program40. One estimate puts the number of people who will have to shop for new coverage at 3.8 million or more40.
Drug prices: lower negotiated prices, higher cost ceilings
UnitedHealthcare's prescription drug pitch comes during big changes to Part D. On Jan. 1, Medicare's negotiated prices take effect for 15 more drugs, bringing the total to 258. Ozempic, Wegovy and Rybelsus drop to $274 for a 30-day supply from a 2024 list price of $9593. Breo Ellipta, Janumet and Tradjenta fall by more than 80%2. CMS expects enrollees to save about $685 million out of pocket in 20271.
Other Part D costs are rising:
- The annual out-of-pocket cap goes to $2,400 from $2,10010.
- The maximum deductible goes to $700 from $6154.
- A federal premium stabilization program worth about $9.8 billion is ending4.
The average stand-alone Part D premium is projected to rise only slightly, from $35.09 to $3614. The drug portion of Medicare Advantage premiums is expected to fall to $7 from $11.3218.
Coverage of the negotiated prices consistently notes that a negotiated price is not the patient's copay. The plan's tier placement, prior authorization rules and coinsurance still decide what a patient pays5. For UnitedHealthcare, that makes the formulary choices behind "$0 Tier 1" more important than the slogan itself. On this, the carrier's marketing and independent consumer advice agree: compare total annual cost, not a single line item.
Medicaid changes make dual-eligible plans more important
The focus on D-SNPs comes as Medicaid enters its biggest change in years. Starting Jan. 1, 2027, many adults aged 19 to 64 in Medicaid expansion programs must show 80 hours a month of work, school, volunteering or job training, or earn at least $580 a month54. Their eligibility will also be reviewed every six months instead of once a year55.
CMS projects about 2.3 million people will lose Medicaid coverage in 2027. Roughly four in ten of those losses are expected to come from paperwork problems rather than failure to meet the requirement53. California alone expects 1.1 million Medi-Cal enrollees to lose coverage by 2029β3052. Arkansas's 2018 work requirement is the main precedent: about 18,000 people lost coverage there with no clear rise in employment58.
Dual-eligible people who are also on Medicare are generally outside the work-requirement population56. Even so, a carrier that markets itself on coordinating Medicare and Medicaid is betting on a Medicaid system under heavy administrative strain.
Employer and hospital costs show the same pressure
The cost pressures UnitedHealthcare cites in Medicare are showing up across commercial insurance too:
- Marsh projects employer health benefit costs per employee will rise 8.2% in 2027 after cost-cutting changes, the highest increase since 2003, and about 11% without them41.
- WTW puts the trend at 11.1% before plan changes43.
- Aon puts it at 9.5%44.
- Business Group on Health puts it at 9.2%44.
The causes cited include higher hospital prices, health system consolidation, GLP-1 drugs, new cancer therapies and AI-assisted billing that increases claim volume and coding levels4648. Workers already pay an average of $7,471 a year toward family premiums, up 9% from 202542. In WTW's survey, 85% of employers are planning or considering shifting more premium costs onto employees by 202843.
Analysis
UnitedHealthcare's 2027 lineup is a retreat to its most profitable kinds of plans, presented as a consumer-friendly upgrade. The core benefits, like $0 primary care and $0 generics, are real, and HMO networks can lower costs for members. But the company is doing what employers and the rest of the Medicare Advantage market are doing: keeping advertised prices low while moving more of the risk onto the people who use the most care, through higher out-of-pocket limits, larger deductibles and narrower networks. Its Medicare Advantage enrollment has already fallen to about 7.5 million from 8.4 million35, which suggests it is choosing margins over growth.
For beneficiaries, the useful comparison is the plan's out-of-pocket maximum plus the $2,400 drug cap plus a year of premiums13. The $12 national average premium tells them very little about what a given plan will cost.
